If you suspect your water supply well may have been impacted by salt run-off please complete and submit via email the sodium chloride damage reporting form. The Glastonbury Health Department is collecting reports of water supply wells and associated fixtures that may have been damaged by sodium chloride run-off in compliance with Public Act 23-31, Section 19. Any reports collected will be submitted to Connecticut's Office of Policy and Management (OPM) in December of each year. The OPM may identify state or federal financial resources to assist owners with the costs of remediation, mitigation or repair. If funds become available OPM will establish criteria and procedures for disbursement of financial assistance.
Be sure to include a water quality report with your form. Submitted reports are confidential and will only be provided to the Office of Policy and Management.
