Summer Camp Forms & Payment Options

Complete your Forms Early!

Many camps have forms that are required to be filled out before you are able to complete the registration process.  This summer, save time and stress on registration day and fill out your forms early!

SUBMITTING A FORM DOES NOT MEAN YOU ARE REGISTERED FOR THE PROGRAM. YOU WILL HAVE TO GO THROUGH THE TRADITIONAL REGISTRATION PROCESS.

Forms that will be available ahead of time are:

Please note that it is not required to fill out forms early, and is available to parents as a convenience. Forms may still be filled out at the time of registration as well.

HOW DO I COMPLETE THE FORMS EARLY?

  1. Click on the form you want to complete above.   
  2. The link will take you to log in to your MyRec account
  3. You will be prompted to select the child you want to complete the form for.  Once selected fill in the form and click “Submit”
  4. You will be brought to that child’s page and should see the custom form listed.  Click “Add Custom Forms” to complete another form for that child.
  5. To complete forms for another child return select “Account” in the Blue Bar in MyRec, then “My Account”.
  6. Click on the next child’s name under “Account Members” to be brought to their page and scroll down to the Custom Forms Section.
  7. On the right side of the page click “Add Custom Forms” and select the form you want to complete.  

HOW TO EDIT OR DOUBLE CHECK FORMS

  1. Go to:Glastonbury Parks, Recreation & Senior Services: Log In (myrec.com)
  2. Once you’ve entered your username and password you should be on your household page, if not click “Account” in the blue bar and then “My Account”.
  3. On your household page click the name of the child you looking to edit under "Account Members", this will bring you to their page.
  4. Scroll further down the page to see the “Custom Forms” sections.
  5. Click “View” or “Edit” next to the right of the form.

IMPORTANT NOTES:

  • If there is a form that is missed, or an incorrect form is filled out, you will be prompted to complete the form at the time of registration.
  • All Parks and Recreation camps will be utilizing the Camper Information Form, the other two forms will be used by camps that go off site (Permission form) or Swimming. These forms will automatically retain the same information and will not need to be completed when registering for other camps that require these forms.

 

PAYMENT OPTIONS FOR SELECT SUMMER CAMPS

In an effort, to better serve our families we are only requiring 50% deposit for the select summer camps.

  • Camp Discovery
  • Camp Discovery Junior
  • Music & Arts Camp
  • Summer Playgrounds
  • Playgrounds Junior

 

PAYMENT OPTIONS: 

  • Pay Full Amount at Time of Registration
  • Pay 50% deposit at the time of registration.  The full amount is due by May 14th.  A late fee of $15 will be charged if you pay on May 15th or after.  If full payment is not received by May 21st your child will be removed from the program and your deposit refunded.
